summary
On June 30, 2021, Ohio EPA conducted a comprehensive desktop review (PCI) of the City of Lima's approved pretreatment program, with facility representatives Ms. Emily Kerber and Ms. Kim Furry present. Key findings included a pending local limit study submitted in February 2019 that had not yet been approved, a discrepancy between the number of industrial users under permits and those listed in the annual report, and an ongoing investigation of surfactants discharged to the WWTP. Three non-binding recommendations were issued regarding the sewer use ordinance, an industrial user inventory, and identification of non-significant categorical users near CSOs.

- number 2
- deadline —
- kind finding

US EPA has a model of a pretreatment sewer ordinance that is very comprehensive and addresses all the current regulations. It can be found here: https://epa.ohio.gov/portals/35/pretreatment/PretreatmentModelSUO.pdf.

⚠ Gaps in the record
- TWIN CAPTURE — not the canonical reading. The portal serves this 2021-06-30 pretreatment desktop review at two docids; edoc-1610423 is the same letter and is canonical for FIDELITY (a scan is read by the vision path alone, with nothing to check itself against). Declared in data/site/document-versions.yaml as a `v2` cluster — the two files are NOT byte-identical.
- The significant_noncompliance field is set to null because no attached form SNC box determination was clearly readable in the provided pages; the checklist pages reviewed show N/A entries for most SNC-related fields for the one IU file reviewed (US Army Joint Systems Manufacturing Center, which was found in compliance).
- Report form shows inspector signature date as 7/15/2021 and reviewer (Thomas Poffenbarger) date as 7/28/21, distinct from both the inspection date (06/30/2021) and the letter date (08/02/2021).
